All 缅因州 Connections Academy students must provide evidence of the 缅因州 state required immunizations. 根据缅因州学校免疫法, children are required to receive certain immunizations in order to attend public elementary and secondary schools. This form may require both the parent/guardian and health care provider signature on one of the following:

Children who are eligible for VFC vaccines are entitled to receive pediatric vaccines that are recommended by the Advisory Committee on Immunization Practices.
